Social Services Coordinator: Part-Time, Milton Adult Day Services/Alzheimer's and Dementia Services of Northern Indiana. Facilitate participant enrollment in Milton Adult Day Services. Identify and provide for each participant’s social, emotional and psychological needs, as they relate to his/her attendance at the adult day center. Assist in developing a plan of care for the continuing development of the participant’s full potential during his/her attendance and time spent at Milton Adult Day Services (MADS) and with discharge plans when they are indicated.
Nutrition Driver: Part-Time, St. Joseph County
This part-time hourly position consists of 5-6 hours per day, 5 days per week, Monday through Friday is responsible for delivering food and supplies to the Nutrition Sites over a specified route(s). Starting times vary depending upon the route to be run or the opening and closing of various Nutrition Sites.
Site Director: Part-time, Heritage Square
The Congregate Meal Program has an opening for a Part Time Site Director at the our new location in Heritage Square Apartments in South Bend. This position reports to the Lead Nutrition Coordinator. The position begins work at 10:00 am to 1:30 pm, which is 3.5 hours, Monday through Friday.

REAL Services has the responsibility of planning, developing, implementing and administering a network of services that are of high standards, are known, available and used by older adults and low income households in north central Indiana (Elkhart, Kosciusko, LaPorte, Marshall and St. Joseph Counties).
The Part-time Transportation Driver is responsible for providing transportation for older adults (who have no other means) to necessary appointments or for errand purposes. Experience working with the elderly is helpful, but not required. You must be available 8:00 a.m. to 5:00 p.m. Average weekly hours are less than 30. You do NOT work on holidays, but you do earn pay for the holiday. You also do NOT work nights or weekends, but you do earn paid vacation and sick time and you can participate in the pre-tax 403(b) retirement savings as well as dental insurance plans. Pay starts at $8.19 per hour. The job is very rewarding, is a great opportunity to make a difference in the lives of older adults, and you can earn a paycheck and some benefits doing it.
You must be able to stand or walk to assist clients. Regular travel is required to assist with transporting of clients within the St. Joseph County service area. You must be able to adapt to the different needs of each client, display patience and respect, help clients in and out of vehicle, assist a client with transportation to and from doctor's appointments, purchase of groceries, perform errands or shopping for clients when they are unable to go, or other assistance with medical items as needed. You may be transporting more than one client at a time and often must balance getting one client home while making sure to get other clients to their appointments on time; therefore time management is very important.
You must possess good driving skills, knowledge of streets in the service area, and ability to work with older adults who may have illnesses or special needs in a sympathetic and patient manner is necessary.
You must have a valid Chauffeur's License, a good driving record, thorough knowledge of state driving laws, a dependable vehicle, and must meet agency required insurance coverage on the vehicle. Your mileage will be reimbursed because you will be driving your vehicle daily. We will check your driving record with the BMV.
