Transportation Driver: Part-Time, South Bend. This position is responsible for providing transportation for older adults (who have no other
means) to necessary appointments or for errand purposes.
Guardian Partner Coordinator: Full-time, South Bend. This position is responsible for helping to maintain the daily operations of the Guardianship Program and. In addition, this position is responsible for marketing the Guardian Partner Program to organizations and individuals in the community.
Weatherization Building Analyst: Full-time, South Bend. This full-time position is responsible for completing audits on eligible homes for heat-loss defects, writing job specifications, and floor plans. This information is provided to sub-contractors who complete the work in each home. This position assesses each home for eligibility. Conducts final inspections as appropriate. The Weatherization Program operates in 5 north-central Indiana counties (Elkhart, Fulton, Kosciusko, St. Joseph and Marshall Counties).
Resource Counselor: Full-time, South Bend. This posision is responsible for initial face to face assessment of client needs and resources. The Resource Counselor establishes services to meet the individual's critical and non-critical needs through the formalizing of supports and coordinating services when needed.
Home Care Manager: Full-time, LaPorte. This full-time position is responsible for addressing client needs and managing the coordination of their on-going care.
Ombudsman: This part-time position is responsible for providing advocacy to residents of nursing homes and assisted living facilities in a portion of Area 2. Area 2 includes LaPorte, Marshall, Elkhart, Kosciusko and St. Joseph Counties.
Certified Nurse/Program Assistant: Part-time/PRN positions at Milton Adult Day Services. This position is responsible to assist with the provision of direct participant services as well as assisting with the planning, coordination, and implementation of individual and group therapeutic activities to all participants. To function as a certified nurse assistant (CNA) under the supervision of a registered nurse.
