In addition, this position is responsible for marketing this program to organizations and individuals in the community. Care Transition Coordinator - South Bend - This full-time position is responsible for the coordination of a new initiative. It will involve working with area hospitals to implement effective care transitions for Medicare recipients in order to eliminate and/or reduce hospital and nursing home readmissions. This position will work closely with hospital staff to assemble and analyze data that will identify key indicators of those who are most at risk of readmission. Part Time Driver -Driver needed to assist elderly clients to various appointments in St. Joseph County. Must be able to grocery shop and do basic vehicle maintenance checks.
