Community and Home Options to Institutional Care for the Elderly and Disabled (CHOICE)
CHOICE funding may be available to all residents of Indiana who have at least 2 impairments in their Activity of Daily Living (ADL) activities. These ADL’s can include impairments with eating, transferring, changing clothes, bathing, managing bowel and bladder functioning, and ambulating activities. A screen will be done by a case manager to verify eligibility for this program.
There are no income limits for the CHOICE program but anyone with over $500,000 in assets does not qualify for CHOICE. Individuals who are at or above a specific income are required to pay a portion of the services received under a cost-share provision of the CHOICE program. Individuals who are at or below 150% of poverty after certain allowable deductions are exempt from cost-share provision, as are any individuals who are on Medicaid. All individuals who are offered CHOICE funds are required to apply for Medicaid regardless of income or assets.
Services available include Case Management, Adaptive Aides, Adult Day Services, Attendant Care, Consumer Directed Attendant Care, Home Health Aide, Homemaker, Home Modifications, Personal Emergency Response System, Respite, Skilled Nursing, Medical Supplies and Transportation.
