Priority Diversion Aged and Disabled Medicaid Waiver
This funding is available to people of any age that are or could be a Medicaid Recipient and are currently in the hospital or Nursing Facility. People who are eligible qualify for nursing facility placement, but opt to delay that placement and instead receive services in their own home.Any nursing home resident, who receives Medicaid benefits, may be eligible to move back to a home setting and receive care paid for by this funding source, if appropriate.
Services available under this program include Case Management, Adaptive Aides and Devices, Personal Emergency Response Units, Attendant Care, Home Delivered Meals, Home Modifications and Respite Care.
