Support Services Medicaid Waiver

This funding source is operated through the Division of Disability and Rehabilitation Services.  Individuals interested in pursuing services under this funding source should contact the BDDS office at 574-232-1412.  This funding is available to people of any age who are a Medicaid Recipient and who have a diagnosis of a developmental disability. Allowable diagnosis can include: Cerebral Palsy, Mental Retardation, Down’s Syndrome, Autism, an injury or accident occurring prior to the age of 22 years old. People who are eligible for this funding source qualify for group home placement, but opt to delay that placement and receive services in the home where they are residing.

To begin the process of placing a referral for this program, please contact  your local Division of Disability and Rehabilitation Services.  The first step to access this funding source is to complete a Long-Term Care Services Application for this Medicaid Waiver.
